Accident Summary:
AT 1600 HOURS ON THE 17TH OF SEP 69, THEY STARTED TO TRANSPORT ARVN TROOPS FROM PZ TO LZ. LT RICHARDSON AND HIS CREW MADE APPROXIMATELY THREE TRIPS INTO THE LZ, TRANSPORTING ARVN TROOPS. THEY THEN DEPARTED AND FLEW TO FIRE SUPPORT BASE NANCY GRID COORD YT566372 TO REFUEL. AT NANCY 1300 POUNDS OF FUEL WAS PUT ON THE AIRCRAFT. THE AIRCRAFT THEN DEPARTED NANCY AND RETURNED TO PICK UP ZONE YT 670 190, WHERE SEVEN ARVN SOLDIERS AND ONE US ADVISOR GOT ON THE AIRCRAFT. THE AIRCRAFT COMMANDER INSTRUCTED TWO ARVN SOLDIERS TO GET OFF THE AIRCRAFT. AFTER THE TWO ARVN SOLDIERS HAD GOTTEN OFF THE AIRCRAFT, LT RICHARDSON PULLED A HOVER CHECK AND PULLED 38 POUNDS OF TORQUE WITH 6600 RPM. THE AIRCRAFT WOULD PULL 41 POUNDS OF TORQUE BEFORE BLEEDING OFF ANY RPM. LT RICHARDSON THEN DEPARTED THE PICKUP ZONE AND RETURNED TO LANDING ZONE, YT 657294. THE LANDING INTO THE LANDING ZONE WAS MADE IN A THREE SHIP TRAIL FORMATION. THE LANDING ZONE WAS APPROXIMATELY 150 METERS LONG, AND 40 METERS WIDE AT ITS WIDEST POINT. THE LANDING ZONE NARROWED IN THE CENTER FORMING A FIGURE EIGHT TYPE LANDING ZONE, WHICH WAS APPROXIMATELY 18 METERS WIDE AT ITS NARROWEST POINT. LT RICHARDSON FLYING INTO THE LZ IN FORMATION, BEGAN TO FALL THROUGH AFTER TRANSLATIONAL LIFT WAS LOST, AND HE THEN STARTED TO LOSE RPM. THE FLIGHT AT THIS TIME WAS AT TREE TOP LEVEL, WHICH WAS APPROXIMATELY SEVENTY-FIVE TO A HUNDRED FEET HIGH. LT RICHARDSON FLYING CHALK #2, ALLOWED HIS AIRCRAFT TO FALL BELOW THE LEAD SHIP THEREFORE FLYING INTO THE LEAD SHIPS ROTOR WASH. AS HIS AIRCRAFT STARTED TO FALL THROUGH, LT RICHARDSON LOWERED THE NOSE AND LOWERED THE COLLECTIVE, IN AN ATTEMPT TO REGAIN TRANSLATIONAL LIFT, AND ROTOR RPM, THIS ONLY INCREASED HIS RATE OF DESCENT. AS THE AIRCRAFT CONTINUED TO DESCEND, LT RICHARDSON FLARED AND PULLED PTICH, IN AN ATTEMPT TO SLOW THE AIRCRAFTS FORWARD MOVEMENT AND RATE OF DESCENT, PRIOR TO TOUCHING DOWN. DUE TO THE ADDITIONAL PITCH BEING APPLIED AND LOW RPM, LT RICHARDSON RAN OUT OF LEFT PEDAL, CAUSING THE AIRCRAFT TO YAW APPROXIMATELY 30 DEGREES TO THE RIGHT. THE AIRCRAFT CONTACTED THE GROUND WITH A SIDEWARD MOVEMENT TO THE LEFT, AT APPROXIMATELY 6 TO 10 KNOTS OF FORWARD AIRSPEED. THE AIRCRAFT THEN BOUNCED INTO THE AIR, AFTER STRIKING A LOG, CAUSING THE AIRCRAFT TO ROLL TO THE LEFT AND THE SYNCHRONIZED ELEVATOR TO STRIKE THE GROUND. THE AIRCRAFT CONTINUED TO ROLL TO THE LEFT COMING TO A REST ON ITS LEFT SIDE AGAINST A STUMP.\\
