Accident Summary:
THE DAY BEGAN AT 0430 WHEN THE CREWS WERE AWAKENED TO GO ON A SCRAMBLE MISSION. AFTER FILING FLIGHT PLANS AND PREFLIGHTING THE AIRCRAFT (DURING WHICH IT WAS DETERMINED THAT AIRCRAFT SERIAL NUMBER 66-1036 HAD NO FM RADIO CAPABILITY) THE FLIGHT PULLED PITCH AT 0530. AFTER MAKING ITS FIRST INSERTION THE SHUT DOWN AT FSB NANCY FOR BREAKFAST. AFTER BREAKFAST THE FLIGHT MADE ANOTHER INSERTION, WENT ON A CONTACT MISSION, AND THEN SUT DOWN AT XUAN LEE FOR LUNCH. ABOUT MIDWAY THROUGH THE MORNING AIRCRAFT SERIAL NUMBER 66-1036 BEGAN EXPERIENCING COMMUNICATIONS DIFFICULTY BETWEEN THE DOOR GUNNER AND THE CREW. AT 1300 THE FLIGHT PROCEDED TO VE DAT WHERE IT PICKED UP A LIFT OF ARVN TROOPS FOR INSERTION IN AN LZ AT GRID COORDINATES YT827490. ENROUTE TO THE LZ THE COMMAND AND CONTROL SHIP (C&C) INFORMED THE FLIGHT THAT THE LZ WAS A ONE-SHIPPER AND THAT THE LANDING HEADING WOULD BE 020 DEGREES. THE FLIGHT WAS ALSO INSTRUCTED TO TAKE UP A SEPARATION OF ONE MINUTE; THIS WAS EVENTUALLY INCREASED TO ONE AND ONE QUARTER MINUTES. THE LZ WAS AN EXTREMELY PRIMITIVE ONE--THE ENTIRE AREA WAS SLOPED, THERE WERE TREE STUMPS SCATTERED THROUGHOUT, THERE WERE TREES ON ALL SIDES, AND THERE WAS A DROP OF AT LEAST TEN FEET OFF THE WESTERN EDGE. (SHOOTING AN APPROACH TO THE GROUND IN THIS LZ WOULD HAVE BEEN EXTREMELY HAZARDOUS AT BEST). LEAD MADE HIS APPROACH BY CLEARING THE TREES, MAKING AN ALMOST VERTICAL HOVER-DOWN, AND COMING TO A HOVER. HE THEN APPROACHED THE GROUND UPSLOPE, PUT THE TREES OF HIS SKIDS INTO THE SLOPE, AND HELD THE REMAINDER OF HIS SKIDS OFF THE GROUND WITH CYCLIC AND COLLECTIVE. CHALK 2 DID THE SAME. CHALK 3 COULD NOT REMEMBER HOW HE HANDLED HIS APPROACH. CHALK 4 MADE HIS APPROACH IN THE SAME MANNER AS CHALKS 1 AND 2. ON HIS WAY OUT OF THE LZ CHALK 4 MADE RADIO CONTACT WITH CHALK 5 (WHO WAS TO HAVE THE ACCIDENT) AND DESCRIBED THE LZ AND SUGGESTED THAT CHALK 5 MAKE HIS APPROACH AS PREVIOUSLY DESCRIBED. CHALK 5, WITH THE PILOT AT THE CONTROLS, MADE THE APPROACH AS SUGGESTED WITH NO DIFFICULTY. THE CREW CHIEF AND THE DOOR GUNNER CLEARED THE PILOT, WARNED HIM OF THE STUMPS, AND THE AIRCRAFT COMMANDER (AC) INSTRUCTED THE PILOT TO KEEP THE TAIL UP. THERE WERE NO PROBLEMS UNTIL THE SEVEN ARVN TROOPS JUMPED OFF THE SHIP AND CHANGED THE CENTER OF GRAVITY SO THAT THE TAIL SWUNG DOWN. THE TAIL STINGER HIT THE GROUND AND THE TAIL ROTOR HIT A STUMP, CAUSING IT AND THE NINETY DEGREE GEARBOX TO BE RIPPED OFF. THE AC IMMEDIATELY GOT ON THE CONTROLS, CHIPPED THE THROTTLE, AND LOWERED THE COLLECTIVE AS THE TAIIL WAS SWINGING TO THE LEFT. THE AIRCRAFT WAS SET DOWN ON A HEADING OF 030 DEGREES AND THE TAILBOOM WAS PUNCTURED BY ANOTHER STUMP. THE REMAINING THREE SHIPS MADE THEIR INSERTIONS INTO ANOTHER LZ APPROXIMATELY ONE THOUSAND METERS AWAY (AS ESTIMATED BY THE PILOT INVOLVED IN THE ACCIDENT.)\\
