Accident Summary:

 THE FLIGHT BEGAN AT 1255 HOURS, 26 JULY 1970, AT SPARTAN HELIPORT. THEY REPORTED TO THE SOCCER FIELD CONTROL AT 1300 HOURS AND WERE TOLD TO REMAIN AT FLIGHT IDLE. AFTER TEN MINUTES SOCCER FIELD CONTROL TOLD ^MR. STAVE^ TO SHUT THE AIRCRAFT DOWN. UPON COMPLETION OF THE SHUT DOWN THE CREW WENT TO THE III CORPS BRIEFING ROOM. AT 1345 HOURS THEY STARTED THE AIRCRAFT AND PREPARED TO DEPART FOR SONG BE RVN. A CALL TO SPARTAN TOWER WAS MADE FOR DEPARTURE INSTRUCTIONS. ^MR. STAVE^ PICKED THE AIRCRAFT UP TO A HOVER, COMPLETED HIS HOVER CHECK, DID A PEDAL TURN AND FACED NORTH. ANOTHER CALL WAS THEN MADE TO SPARTAN TOWER FOR DEPARTURE CLEARANCE. SPARTAN AGAIN ADVISED THAT THE WINDS WERE LIGHT AND VARIABLE AND OUT OF THE NORTHWEST AT FIVE KNOTS. HE WAS CLEARED FOR A 330 DEGREE DEPARTURE. A PRE-TAKE OFF CHECK WAS COMPLETED BY LT HOLLOWAY AND HE REPORTED ALL INSTRUMENTS IN THE GREEN. THE DEPARTURE WAS THEN EXECUTED IN A NORMAL MANNER. AS THE AIRCRAFT REACHED AN ALTITUDE OF FIFTY TO SIXTY FEET THE AIRCRAFT EXPERIENCED A LOSS OF POWER. A DECELERATION WAS ENTERED IMMEDIATELY. THE AIRCRAFT IMPACTED TAIL BOOM FIRST AND IT BROKE OFF. THE MAIN FUSELAGE OF THE AIRCRAFT CONTINUED IN A FORWARD MOTION AND IMPACTED HARD ON THE HEEL OF THE SKIDS. THIS CAUSED THE AIRCRAFT TO ROCK FORWARD AND UP ON ITS NOSE. AT THIS TIME THE MAIN ROTOR STRUCK THE GROUND AND CAUSED THE FUSELAGE OF THE AIRCRAFT TO TURN 180 DEGREES AND LAND INVERTED FACING SOUTH. ALL THE PERSONNEL ON THE AIRCRAFT EXITED AND WERE TAKEN TO THE DISPENSARY, EXCEPT ^MR. STAVE AND LT HOLLOWAY^ WHICH WERE FLOWN TO THE ^93RD EVAC HOSPITAL^ AT LONG BIEN, RVN.\\
